    MDS COORDINATOR

    The MDS Coordinator is responsible for coordinating and managing the MDS process in accordance with all federal and state government guidelines. 

     

    Qualifications/Education/Experience

    • Must be a Kansas Licensed LPN or RN 
    • Experience in MDS 3.0 and skilled nursing, prefer  1-2 years in  Long Term Care 
    • Working knowledge of Medicare, RUG's, federal and state regulations

    Job Duties/Responsibilities

    • Serves as a professional member of The Cedars interdisciplinary team responsible for providing a safe, home-like environment and quality of life for residents. Ensures that this team completes a comprehensive Resident Assessment Instrument (RAI) that includes Minimum Data Set (MDS).
    • Evaluates residents regarding criteria for Medicare coverage, including clinical eligibility and availability of Medicare days.
    • Ensures all necessary written documents such as Admission Nursing Assessments, Nursing Notes, Overall Care Plans, and other required information is kept current in the resident record system.
    • Maintains a calendar of MDS assessments to be completed and provided to social services for scheduling care plan meetings; attends/participates in care plan meetings.
    • Assists Director of Nursing in keeping facility standards up to federal, state and ASI requirements.  Assists with all inspections. 
    • Assists with the admission process 
    • Works with the Director of Nursing and team on care plan interventions that support resident needs and safety
    • Is available on call when needed by staff on duty for nursing care.
    • Completes assigned sections of MDS including CAAS summaries.   
    • Review and update care plans, complete and update all required assessments and participates in Medicare meetings and Care Plan meetings.  
    • Transmits RAI data to CMS in accordance with guidelines

    every single day.
